Prior to Your Visit

Thank you for choosing Kansas City Cardiology for your care. Please see below for the items to bring with you to make your first appointment.

• A valid insurance card
• A Photo ID
• Medication list with dosages OR please bring your actual medication bottles
• Please print and complete the downloadable PDF forms, or arrive 15 minutes early to complete in the office
• Referral: If your insurance plan requires a referral it is your responsibility to make sure our office has one on file. (This may be in paper or electronic form). You may call your Benefits Phone Number located on your insurance card prior to scheduling your procedure.)
• Co-pay: Your office visit co pay is due at the time of service.
