Appointments

How do I book an appointment?
Schedule an appointment by calling 816-228-1000 or use our appointment request feature on our website. We will reach out to you on the same day or the next day.

Cancellations

Cancellation Policy
Your appointment time is reserved especially for you. If you must cancel your appointment, please call our office at least 24 hours prior to your scheduled appointment. This us to offer that time to another patient. If a patient does not cancel an appointment 24 hours in advance or fails to show up for their appointment, there will be a $25.00 fee charged to the patient account.

Care Manager

What is a Care Manager?

A care manager is a Nurse who specializes in providing support to individuals in conjunction with patient’s goals.

Some of a Care Manager’s patient-centered responsibilities are:

  • Manages and follows a designated caseload of patients within the associated practice
  • Provides targeted interventions to avoid hospitalizations and emergency room visits
  • Coordinates care across settings and helps patient/families understand health care options

Additional ways the Care Manager can help you as a patient are:

  • “Check in” calls to assess health care, educational, and psychosocial needs of the patient/family
  • Develop comprehensive individualized plan of care and targeted interventions (goals), in partnership with the provider
  • Self-management support (i.e. DM, HTN, Obesity, etc.)
  • Explore and resolve uncertainties about health conditions
  • Identify qualifying patient financial assistance programs for DM medications

How much is a visit if I don't have insurance?
Depending on if you are a new patient, established patient, and what you are being seen for the price can vary. Please call our office at: (816) 228-1000 and we will be more than happy to give you an estimate over the phone.

Emergencies

Medical Emergencies

If you are experiencing a true medical emergency, please call 911 or go directly to the nearest hospital. If you need to speak to a physician after hours, and it does not pertain to rescheduling a visit or refilling a prescription, please call our office at 816-228-1000 and you will be directed to the physician on call.

We invite you to call our office to verify your insurance coverage and any payment that you’ll be responsible for at your appointment.

New Patients

What to expect at your first visit?
During your initial visit, you will be seen by our staff, who will ask about your medical history and your symptoms. After reviewing your medical history and diagnostic tests, you will be presented with a number of treatment options. We will inform you of the risks and benefits of each option and will assist you in making the final decision concerning which option is best for you.

“No-Show” Policy

What is the "no-show" policy?
We ask that patients call 24 hours prior to their appointment if they need to cancel their appointments. Please call our staff to inform them of your need to cancel or reschedule as soon as you are able. If an appointment is missed without an advance phone call, additional fees may apply.

Prescriptions

Prescription Refills
All prescriptions will be refilled during normal business hours. We do not refill prescriptions after hours or on weekends. If you are out of refills and our physicians need to see you before prescribing more medication, our office will schedule an appointment for the first available date.

Clinician

What clinician can I see?

We ask that you see your primary care clinician for your annual wellness visits and maintenance visits as they are the ones most familiar with your plan of care. If you have an urgent need, we are more than happy to get you in with any of our clinician that are available. We are one clinic, one team.

Same Day Appointment

Can I be seen same day?
Absolutely. We do everything in our power to ensure we have urgent slots available for patients who have acute, urgent health concerns. This includes, fevers, chest congestion, diarrhea, etc.

Timing

How early should I arrive to my appointment?
When you make your appointment, our schedulers will instruct you how early to arrive. We ask that new patients and annual appointments arrive at least 30 minutes early to ensure that we have enough time to update all information. This is especially true for Medicare Annual Wellness visits. There is quite a bit of preparatory work for these appointments and it is important to have this completed before the provider sees you for your visit. Completing the required preparatory work before you arrive if possible, or arriving early enough to complete it helps ensure smooth and on time care for all patients.

What To Bring

What should I bring with me to my appointment?
Please be sure to bring your insurance card, driver’s license or photo ID, copay, current medications or medication list, and any questions you have for your provider during your visit.
